Sample Confidential Personal Identifier Forms
Under Sup. R. 45(D)(1) and (2), a party submitting a case document to a court or filing a case document with a clerk of court must omit personal identifiers from the case document and include that information on a separate form (i.e., a "personal identifier form") for use by parties. The following are personal identifier forms prepared by various Ohio courts and clerks.
