Uniform Ohio Domestic Relations Forms, Affidavits 1-5
The Supreme Court of Ohio, pursuant to Ohio Rule of Civil Procedure 84, approved on May 25, 2010 the following Uniform Domestic Relations Forms (Affidavits 1 through 5), which are effective July 1, 2010. The forms are part of the “Appendix of Forms” attached following Rule 86 of the Ohio Rules of Civil Procedure.
The forms are available, by clicking on the links below, in an interactive PDF format, so they may be completed online and printed and/or saved. The PDF files also may be downloaded to your computer. It is strongly suggested that you download the latest version of Acrobat Reader so you will be able to save partially completed forms for later completion on your system.
