If an assignment lasts more than one day, list each date on a separate line. Reports must be completed no later than the end of the calendar month following the calendar month in which the work was performed. When the dates of service span two calendar months, the report must be completed no later than the end of the calendar month following the calendar month of the earliest date of service.  See Guidelines for Assignment of Judges, Guideline 6.2.(B).
